***Up-to-date Sports Physical Required***
IMPORTANT: An up-to-date doctor-signed physical is required to complete fall sports registration. Updated physicals must be uploaded into Annasis as a part of the registration process. Please reference the 2026 Fall Sports Physical Exam Eligibility Check page to verify if your child's physical is up to date.
The physical must be:
- Dated within two years of the last day of the season (for most, that means on or after November 1, 2024)
- Signed by a licensed medical provider
- Uploaded by you during registration (if an updated version is required at this time)
Thousands of athletes are currently booking physicals right now, so please do not delay! Your child will be unable to participate in Oaks athletics until this is submitted.
